The Canadian Red Cross Society, a non-profit, humanitarian organization dedicated to helping Canadians, as well as the most vulnerable throughout the world, is seeking in the Sault Ste. Marie and surrounding area. Reporting to the Volunteer Coordinator, the Health TAPESTRY Volunteer will work to help people stay healthier for longer in the places where they live. The Health TAPESTRY Volunteer will work in this adaptable model of health care to connect senior clients with their primary health care team and their community, through the support of technology. Key Responsibilities • Visits clients in their homes to conduct wellness assessment surveys/questionnaires • Assists clients in personal health-related goal identification • Provides social connection to older adults • Communicates with volunteer organizations, family health team, research team, and/or community based organizations • Utilizes a tablet computer and other technology to provide information to identified organizations • Maintains up to date knowledge of health programs and services and acts as resource in the community through the use of approved information and referral sources • Works closely with program coordinator to schedule client appointments • Completes feedback and reports; and submits information to the volunteer coordinator Working Conditions • Visit clients at their home (average visit time from 1 to 2 hours) - the conditions of the client’s home may not be known prior to the visit occurring • All visits will be within the assigned community • Local travel is required. Must have reliable transportation by personal or public transit • Meetings and supervision may be in person, by telephone or through other electronic means • Must have access to a telephone, email and internet • Time spent engaging in activities will vary by a volunteer’s interest and availability with the average person committing 6 hours per month
